Untangling the Web: Systems Approach for Healthcare's Wicked Problems and Digital Solutions
In the wake of the COVID-19 pandemic, the healthcare landscape has evolved into an even more intricate ecosystem, presenting a series of "wicked problems" - complex challenges that resist simple solutions. The unprecedented exodus of healthcare workers has exacerbated the preexisting manpower crunch, creating a perfect storm of interconnected challenges. While the industry has turned to digital technology as a potential panacea, this shift has inadvertently introduced new gaps and disconnects within health systems. Healthcare practitioners find themselves in a paradoxical situation: armed with first-class technology, yet unable to fully harness its potential. 
 
As we navigate this complex terrain, many practitioners grapple with a pressing question that exemplifies the nature of wicked problems in healthcare: How can we leverage advanced digital tools like IoT, AI, and machine learning to enhance efficiency and job satisfaction without being overwhelmed or replaced by technology?
 
This seminar brings together a distinguished panel of experts in systems and complexity thinking for health system transformation, dynamic modelling and simulation, and practical implementation of both systems thinking and technology in the community. Participants will gain invaluable insights and be introduced to practical tools and applications that can be immediately applied to initiate small-scale digital innovations and improvements within their organisations.
 
Moderated by the Chief of Digital Health, the seminar will feature an interactive and engaging panel discussion. This session promises to unravel the intricate web of systemic complexity, offering digital solutions that are both powerful and accessible. Following the panel discussion, participants will be invited to a networking session over refreshments where like-minded participants can exchange ideas, share experiences, and forge valuable connections. It's your chance to discuss potential collaborations, gain fresh perspectives, and continue the dialogue on transforming healthcare digitally.

A/Prof Ng Yeuk Fan will share his experiences about systemic healthcare/health systems transformation, and discuss its implication on data, informatics, and digital transformation. Effective whole systems transformation requires a comprehensive approach to data, informatics, and IT models that are based on person-centred, integrated and value-based models of care. This perspective of designing a cascade of “interdependent systemic models” to help us to accelerate care and digital transformation coherently will help us to attain better outcomes and value for patients and populations sooner. Dr Ng will also be emphasising the need to take a polarity-based approach, where we address immediate challenges while working systemically towards our envisioned future goals.

Digital health technologies have demonstrated promising results in controlled trials, yet bridging the gap between laboratory efficacy and real-world effectiveness remains a significant challenge. This talk examines the complex landscape between digital health's theoretical potential and its practical impact in healthcare settings. We explore critical barriers to implementation, including user adoption challenges and system integration hurdles. Our discussion particularly focuses on decision-making complexity in healthcare environments, examining how different stakeholders—from developers to clinicians—evaluate and influence both the adoption and economic value of digital health solutions throughout their development and clinical validation process.

 2:40pm Systemness in Healthcare: Harnessing Systems Approaches to Navigate Uncertainties and Complexities

We are at the cusp of an age buffeted by forces of volatility, uncertainty, complexity, and ambiguity (VUCA). It is not surprising that wicked problems are top-of-mind for policymakers and researchers. Ageing, the chronic disease burden, and the threat of future epidemics are known challenges; we know that we are subject to “black swan” events. On the other hand, technological innovation has opened an array of potential solutions to address long-standing health system challenges. Why should we enhance the systemness of our health systems? How can we achieve systemness with digital health and systems approaches? What are some of the potential opportunities and challenges to address the uncertainties and complexities in modern health systems? Associate Professor Sean Lam, head of the data science pillar at SingHealth, will address these questions based on his 20+ years of working in health services and systems research.

Shailu Suresh is the Chief of Digital Health Practice at the NUS-ISS. With 25 years of collective work experience in Singapore and India, in various fields, such as wet bench research, research management, lecturing, hospital operations and management, in both private and public healthcare sectors,

Shailu has more than 14 years of working experience in the healthcare and biomedical sciences domain in Singapore. She has held senior management and administrative posts at various government and statutory boards including overseeing hospital operations for 4 years at Mount Elizabeth hospital and heading Gleneagles hospitals in Bangalore, India, as the CEO for 2 years. In her role as the CEO for the hospitals in India, she was also engaged in training the hospital staff in service excellence and productivity improvement, using tools such as Kaizen and 6-S.

Apart from hospital operations, during the 2020 COVID-19 pandemic, she led and managed the ground operations for Singapore’s Covid-19 National Testing and Operations Task Group as the Senior Deputy Director at Health Promotion Board. In this role, she had complete oversight of the swab operations’ end-to-end workflow and was in charge of the overall planning, developing standard operating procedures and co-ordinating with various whole of government (WOG) sector leads to ensure smooth swab operations throughout Singapore.

About 10 years of her working life has been in the areas of biomedical research and academics, teaching subjects such as Biochemistry, Human Anatomy and Physiology and Research Methodologies, at Nanyang Polytechinc and MDIS respectively.

Shailu and her team will be working towards accomplishing the Digital Health's mission of transforming the future experience of health through technology, data and design.

Sean Lam is the deputy director for the SingHealth Health Service Research Centre. He is also an Adjunct Associate Professor with the Duke-NUS Health Service and Systems Research Signature Program and the SingHealth Duke-NUS Global Health Institute. His research passion lies in the application of decision science and quantitative methodologies to solve complex health system problems. His interdisciplinary research focus aims to advance academic knowledge and influence healthcare policy and practice, promoting a data-driven, evidence-based, and patient-centric approach towards the effective delivery of health services.

Dr Ng Yeuk Fan is a Senior Consultant Public Health Physician experienced in health systems and services planning and evaluation, healthcare corporate transformation, organisation development and change management.

He is currently Director & Head of Corporate Development in Yishun Health, a member of the National Healthcare Group. As the Director, he is responsible for value based care & services planning, health system & services evaluation, insights & analytics, corporate planning, and organisation development. Ongoing focus areas include the development and deployment of a Unified Care Model for the Yishun Zone Regional Health System in NHG, development of a health system and health services performance measurement framework, value-based transformation of lifelong care and episodic care services, development of person-centered needs assessment tools, implementation of needs-based population segmentation tool, development of population outcomes and financial risk prediction models, deployment of population health survey for the North of Singapore, development of a unified data model for population health systems, and development of organisation development methods to accelerate health system transformation.

Dr Ng is an Adjunct Associate Professor in the Saw Swee Hock School of Public Health, as well as Adjunct Associate Professor in the Health Systems & Services Research Program in the Duke-NUS Medical School. He is also a physician faculty in the National Preventive Medicine Residency Programme in National University Health System, and a clinical teacher in the Lee Kong Chian School of Medicine.

His academic interests include the application of systems thinking, design thinking and complexity thinking in health systems, value based care, integrated care, population health systems and health system transformation.

Yoann Sapanel is a pre-eminent expert in bridging payer perspectives with medical technology development. He is the Head of Health Innovation, at the Institute for Digital Medicine (WisDM) at the Yong Loo Lin School of Medicine, National University of Singapore, where he accelerates cross-industry, public–private collaborations to validate, implement and scale first-in-kind pharmacological and digital technology into the clinic. Yoann co-authored the best-selling book, Medicine Without Meds, published by Johns Hopkins University Press.

Mr Glenn Neo

Mr. Glenn Neo is the Director of Innovation Capabilities Enablement (ICE) of Synapxe Pte Ltd. 
As the Director for ICE, Glenn leads his team to develop innovation strategies and capabilities to accelerate HealthTech exploration and experimentation, as well as establish and manage programmes and platforms that spur innovation through collaboration. The alliances formed bridge across the healthcare and technology sectors, and bring together dynamic resources to advance smart health innovations aimed at transforming population health. 

Glenn was the past-immediate Chief Information Officer of Woodlands Health (WH). In the six years he held this role, Glenn led the delivery of more than 60 IT infrastructure, applications and innovation projects to improve patient care and care productivity. The 1,800-bed campus, with fully integrated acute and community hospitals, specialists’ outpatient clinics, and an intermediary & long-term care facility, will reinvent the way healthcare is delivered through data analytics, artificial intelligence and smart technologies for better health outcomes. 

Before joining Singapore public healthcare, Glenn has held senior roles in consulting, systems integration and angel-investing businesses - across the public sector, defence engineering, supply chain logistics and financial services domains in the Asia Pacific region.
