In an increasingly interconnected and fast-paced world, health and social care system will struggle to keep up with the challenges brought upon by rising ageing issues if they continue to use traditional approaches. Enter the era of digital transformation—a disruptive force that promises to reshape health and social care delivery and social support services.

Join us in this sharing session as our speakers share their insights and experience on their digital transformation in their community and how they could potentially tap on the Community Care Digital Transformation Plan (CCDTP) for their journey;

The panel will be moderated by Mr Bennie Chia, Assoc Lecturer and Consultant, Smart Health Leadership Centre, NUS-ISS and will feature speakers from a variety of backgrounds, including:

1.bennie 

Mr Bennie Chia

Assoc Lecturer and Consultant, Smart Health Leadership Centre, NUS-ISS
View Biography

Bennie is currently an Associate Lecturer & Consultant, Smart Health Leadership Centre with NUS-ISS. Through his wealth of experience in healthcare, Bennie led numerous national and institutional level projects and contributed vastly in areas such as healthcare operations, service innovation, training delivery and student development.
Bennie was involved in the development and implementation of the national plan on caregiving and community mental health. His works in the community sector includes identifying, developing, enhancing capacity and capability and piloting initiatives to enhance and increase mental health (especially dementia) awareness through outreach, care coordination with the health and social care service providers.

Prior to joining NUS-ISS, Bennie spent a couple of years honing his knowledge as a Gerontologist practitioner by developing, revamping and delivering related curriculum through blended learning to both youths and adult learners.

Bennie hopes to drive changes in the health and community sector through his passion and support for healthcare transformation and improvement to quality of life.

Mr Adrian Tan

Adrian Tan is a compassionate advocate and the COO of SG Assist, a dedicated organization committed to making a positive difference in the lives of caregivers and seniors. Inspired by his personal journey of caring for his mother with depression, Adrian made a significant sacrifice by leaving his flourishing career to co-found SG Assist. Adrian's own experiences as a caregiver shaped his vision for SG Assist and his relentless pursuit of a more inclusive and compassionate society.
 
Adrian's deep understanding of caregiving needs and his unwavering commitment to improving the lives of caregivers led to the creation of the CareConnect platform, empowering caregivers with the resources and support they need. Additionally, Adrian has been instrumental in establishing the Caregiver Resource Centre, the first of its kind in Singapore. The Caregiver Resource Centre serves as a vital touchpoint for caregivers, providing hands-on experiences with technology solutions and offering a wide range of support and resources. Adrian's vision for the Caregiver Resource Centre has transformed it into a nurturing space that equips caregivers with the knowledge and tools to enhance their caregiving journey, fostering a sense of community and empowerment.

4.farouk 

Mr Mohamed Farouk Bin Mohamed Ismail 

Director, IT, Innovation & Digitalisation Division, Agency for Integrated Care

Mr. Mohamed Farouk has been with AIC since July 2019, in which he led the planning and development of services and resources to support caregivers of the elderly.

He is currently the Director, Sector Digitalisation & Innovation where he oversees AIC’s support of its primary care and community partners in their adoption of Digital and Technology to achieve their strategic goals, enhance operational effectiveness, and support the development and implementation of IT systems in the Community Care Sector to better care for clients. He was previously with the Accounting & Corporate Regulatory Authority and the Ministry of Manpower overseeing customer experience and operations respectively. 

He has a Bachelor’s Degree in Political Science from the National University of Singapore and a Master in Public Management from the Lee Kuan Yew School of Public Policy and the Harvard Kennedy School.
