Consent for Collection, Use & Disclosure of Personal Data

At the Institute of Systems Science, National University of Singapore (“NUS‐ISS”), the trainee’s personal data will be used for the purposes of managing and administering his/her course, examination and certification (i.e. the Programme).

The trainee’s personal data will include data such as (a) Name (b) NRIC/FIN/Passport Number (c) Nationality (d) Date of Birth (e) Race (f) Highest Education Attained (g) Job Title (h) Company Name (i) Salary Range (j) Contact Numbers and (k) Email Address.

They may be disclosed to relevant organisations that necessarily require such personal data, such as NUS‐ISS course partners, certification awarding bodies, examination agencies and funding public agencies. The personal data may also be disclosed to third party service providers engaged by NUS‐ISS for the purpose of conducting course‐related surveys required by the funding public agencies.

The personal data will be retained by NUS‐ISS for verification requests from your employer and funding public agencies, until such time the purpose for the personal data and any other legal or business purpose are no longer served by the retention of such personal data.

If you are enrolling yourself for the Programme, the above applies to the consent for the use of your own personal data for the above mentioned purposes.

If you are enrolling your colleague on behalf of the organisation for the Programme, then the organisation declares and warrants* it has, prior to disclosing the personal data of the trainee(s) to NUS-ISS, obtained the appropriate consent from the trainee(s) for the collection, use and disclosure of their personal data for the purpose of participating in the Programme.
